چکیده

Although faintness, syncope, and sudden death are common in hypertrophic obstructive cardiomyopathy, their cause has not been well documented. The occurrence of ventricular asystole causing syncope after exercise is described in one patient. The elucidation of the disturbance of rhythm may, therefore, be ofgreat importance, as sudden death may be avoided in these patients by the prophylactic use of a pacemaker.

Hypertrophic cardiomyopathy and tachyarrhythmias detected by a pacemaker.

A 56-year-old man came to the Pacemaker Clinic for his regular pacemaker control. He had experienced a syncopal episode in the previous week. He had a previous diagnosis of non-obstructive hypertrophic cardiomyopathy. Due to a previous syncope and documented 2:1 infrahisian block, a dual-chamber permanent pacemaker had been implanted a few years before. The device was interrogated, showing seve...

Percutaneous Septal Ablation in Hypertrophic Obstructive Cardiomyopathy: From Experiment to Standard of Care

Hypertrophic cardiomyopathy (HCM) is one of the more common hereditary cardiac conditions. According to presence or absence of outflow obstruction at rest or with provocation, a more common (about 60-70%) obstructive type of the disease (HOCM) has to be distinguished from the less common (30-40%) nonobstructive phenotype (HNCM). Symptoms include exercise limitation due to dyspnea, angina pector...

Impact of Dual-Chamber Permanent Pacing in Patients With Obstructive Hypertrophic Cardiomyopathy With Symptoms Refractory to Verapamil and j3-Adrenergic Blocker Therapy

Background. Patients with obstructive hypertrophic cardiomyopathy (HCM) with symptoms refractory to drugs (j3-blockers or verapamil) are candidates for cardiac surgery (left ventricular septal myectomy or mitral valve replacement). The present study examines prospectively the ability of dual-chamber (DDD) pacing to improve symptoms and relieve left ventricular outflow obstruction in such patien...

Coil Embolization of Septal Branches in Hypertrophic Obstructive Cardiomyopathy

Hypertrophic cardiomyopathy (HCM) has been associated with sudden death in young athletes. On long-term follow up, syncope, chest pain and dyspnea are well known frequent symptoms due to the dynamic left ventricular outflow tract (LVOT) obstruction, mitral regurgitation and diastolic dysfunction. To reduce the LVOT pressure gradient, septal myectomy and alcohol injection have recently been util...
